For Providers who want data and insights to self manage their AN-ACC function and for PA to take care of the identification of funding opportunities and provision of resident priority lists to embed in their AN-ACC workflows.

Clinical Care Coordinator: To complete assessment, documentation and care planning placement to maintain compliance in documentation and to maintain funding.

Tell us about the challenges being addressed: 
At Central Gippsland, we have experienced a shortage of clinical staff in aged care. This has impacted on our ability to train new staff to our desired standard of assessment and documentation to support compliance and AN-ACC funding.
 

Tell us about the solution put in place:
PA has been valuable in identifying, preparing for, and facilitating resubmissions which have resulted in increased funding for our service.

We are now looking to work with PA to develop training for our staff, and update our internal procedures to align with AN-ACC assessments, minimising duplication of assessments.

Tell us about your experience:
PA has supported all aspects of our service delivery, enabling us to maintain our compliance and revenue, minimising any financial loss and risk to the business.

Will you continue to utilise the Partner in Care service?
Central Gippsland will continue to utilise the Partner in Care service for care coordination, staff education, medication and GP rounds, if required. PA is our preferred organisation for AN-ACC support, education and guidance.
