How do I submit a Reclassification in My Aged Care?

Submitting Reclassifications (also known as Requesting a Reassessment): How does it work?

With Reclassification one of the most important things that Providers should be doing in the lead up to the implementation date of AN-ACC, here is what you need to do to be able to submit your reclassifications in My Aged Care.

Definitions

What is a Reclassification (also known as a Reassessment)?
A Reclassification or Reassessment should be requested when a Resident’s situation, needs or circumstances have changed.

What is a Reconsideration?
If you do not agree with the AN-ACC Classification that has been assigned to a Resident, you can request a Reconsideration within 28 days of the AN-ACC Class being assigned.

Requesting a Reclassification: How To Guide

1. Log into the My Aged Care Provider Portal
2. From the Home Page, click on the Residential Care tile

3. The screen will show the list of Residents under the Care Recipients

4. Select the Resident who you wish you submit a Reassessment request for
5. Once you are in the Resident file, select the Residential Care tab at the top of the screen.
6. From this screen, you can view the Resident’s current Permanent  and/or Respite Classification & Classification history.
7. To request a new Classification, click the button underneath the relevant current Classification (Permanent or Respite) that says Request Reassessment.

7. Confirm Yes if there has been a significant change in needs and then select Confirm Reassessment.

*** If there is a Permanent Resident Reclassification request that urgently needs attending to (i.e. Palliative Residents declining quickly and Assessors not getting out in time), there is now an option to submit an urgent Reassessment request via My Aged Care. You can request an urgent Reassessment:

  • if the care recipient is at imminent end of life, and;
  • has a documented care plan in place that has been communicated with the resident and their family and/or carers.

 

When an urgent request has been submitted, you will also need to contact ANACCassessments@health.gov.au, identifying that the resident is approaching end of life and requires an urgent reclassification assessment.***

8. For Permanent Residents, you will be required to select the criteria that your Resident meets for Reassessment:

  • Care Recipient has been an in-patient of a hospital for at leave 5 days consecutively
  • Care Recipient has been an in-patient of a hospital for at least 2 days and was administered general anaesthetic during this period
  • For a Care Recipient with an exisiting Classification between 9 and 13, at least 6 months have passed since the result of the exisiting Classification
  • For a Care Recipient with an exisiting Classification level between 2 and 8, at least 12 months have passed since the result of the existing classification
  • The condition of the Care Recipient relating to mobility, cognitive ability, function, pressure sore risk and / or compounding factors has changed

 

You can enter supporting evidence in the free-text section under Further Justification.

You may see that some options are greyed out and not selectable – this will occur when time-based criteria has not been met.

9. For Respite Residents, you can select from the following criteria to meet Reassessment:

  • Condition of Care Recipient has changed from independently mobile to being mobile only with assistance
  • Condition of Care Recipient has changed from independently mobile to not mobile
  • Condition of Care Recipient has changed from mobile with assistance to not mobile

For Respite Reassessment requests, you must also add in an expected Departure Date.

You can enter supporting evidence in the free-text section under Further Justification.

10. Once criteria has been selected and you have completed your supporting documentation under the Further Justification section, hit Request Reassessment!

Important to note: As specified above, if there is a Reclassification request that urgently needs attending to (i.e. Palliative Residents declining quickly and Assessors not getting out in time), an urgent Reassessment can be requested via the Reassessment Request process in My Aged Care, with an email also to be sent to ANACCAssessments@Health.gov.au.
